The Four Documentation Items That Can Stop a Job

Physical controls are visible. Documentation gaps are not — until an inspector or owner’s rep asks for them. In my experience these four items account for a disproportionate share of stop-work orders and citations that had nothing to do with an actual unsafe condition.

1. Competent person designations

OSHA 1926 requires a competent person — someone capable of identifying hazards and with authority to correct them — for scaffolding, excavation, fall protection, cranes, and more. The requirement is not a title. It is a documented designation, backed by training records, for each specific hazard category on each site. A common finding in practice: one name covers every category on every project, with no evidence that person was on site.

2. Site-specific job hazard analyses

Where a JHA or JSA is required by the employer, owner, contract, or applicable safety program, it needs to reflect the actual work being performed. A JHA written for a previous project, or a generic template that never names the current tasks and controls, leaves a significant documentation gap. Owner safety reps in particular check for scope drift — the JHA describes rebar tying, the crew is now doing overhead welding.

3. Training records that match the work

Fall protection, scaffold user, confined space entrant and attendant, silica awareness, powered industrial truck, and rigging/signal person training all need to be documented and current. The check is simple: pull a name off the sign-in sheet and ask for the certificate.

4. OSHA injury and illness recordkeeping

If your company had more than 10 employees at any time during the previous calendar year and the establishment is not partially exempt by industry, OSHA recordkeeping requirements generally apply: maintain the OSHA 300 Log, complete the 300A summary, and post the 300A from February 1 through April 30 each year — even with zero recordables. Certain establishments also have electronic submission obligations through OSHA’s Injury Tracking Application, based on establishment size and industry; construction is among the industries covered by the 20–249 employee Form 300A submission requirement. The requirements are published by OSHA under 29 CFR 1904. A recordkeeping violation can result in a citation even when the underlying physical conditions are otherwise safe.

⚠️ Common finding in practice: the 300A is not posted, is posted late, or is not certified by a company executive as required. It is a paperwork violation, but it is a citation.

Jobsite Hazard Checklist: The Focus Four and Beyond

OSHA identifies the Focus Four — falls, struck-by, caught-in or -between, and electrocution — as the leading causes of construction fatalities. Your checklist should give these hazards prominent attention. The full text of 29 CFR 1926 is available on OSHA.gov.

Fall protection (Subpart M)

  • ✅ Fall protection provided where required by 1926 Subpart M or the task-specific construction standard — for general Subpart M applications, unprotected sides and edges 6 feet or more above a lower level (guardrails, safety nets, or personal fall arrest)
  • ✅ Personal fall arrest anchorages meet the applicable 1926.502(d)(15) requirements and are suitable for the complete system in use — not “looked strong enough”
  • ✅ Harnesses and lanyards inspected before each use, with documented periodic inspection by a competent person
  • ✅ Holes and skylights covered and marked; leading edges controlled
  • ✅ Prompt rescue or self-rescue capability established before personal fall arrest systems are used

Scaffolds and ladders (Subparts L and X)

  • ✅ Scaffold erected under a competent person and inspected before each shift
  • ✅ Fully planked platforms, guardrails above 10 feet, safe access provided
  • ✅ Ladders inspected, secured, and rated for the load; nonconductive siderails used where contact with exposed energized electrical equipment could occur
  • ✅ Extension ladders used for access extend at least 3 feet above the landing and are set at approximately a 4:1 angle

Excavation and trenching (Subpart P)

  • ✅ Protective system (sloping, shoring, or shielding) in place at 5 feet or deeper
  • ✅ Daily competent person inspection, and re-inspection after rain or any change in conditions
  • ✅ Spoil piles and equipment kept at least 2 feet from the edge
  • ✅ Ladder or ramp within 25 feet of lateral travel for trenches 4 feet or deeper
  • ✅ Utilities located and marked before digging

Struck-by and caught-in (Subparts N, O, CC)

  • ✅ Crane operator certification and signal person qualification documented
  • ✅ Rigging inspected before each use; load charts on the machine
  • ✅ Swing radius barricaded; spotters for backing equipment
  • ✅ Hard hats, high-visibility apparel worn in all equipment zones
  • ✅ Machine guarding and LOTO applied to fabrication equipment brought to the field

Electrical (Subpart K)

  • ✅ GFCI protection or an assured equipment grounding conductor program for all temporary power
  • ✅ Overhead line clearances maintained for cranes and lifts
  • ✅ Energized work justified in writing and performed under an electrical safe work program
  • ✅ Extension cords rated for hard usage, inspected, and removed from service when damaged

If you are a fabrication shop sending crews into the field → the controls your shop takes for granted, especially LOTO and machine guarding, do not automatically travel with the equipment. Field LOTO for construction is governed by 1926.417 and is frequently thinner than the general industry program back at the plant. The ANSI/ASSP Z244.1 lockout/tagout standard provides the program structure OSHA’s construction rule does not, and it applies whether the equipment is bolted to your shop floor or on a trailer.


Health Hazards Inspectors Increasingly Prioritize

The Focus Four still drive fatality statistics, but health-hazard enforcement has grown. Three items belong on every construction compliance checklist in 2026:

⚠️ For coatings and welding crews specifically: respiratory protection is where I have seen the most inconsistency between shop practice and field practice. A fit test performed at the plant does not cover a different respirator model handed out in the field.


Consensus Standards Owners and Inspectors Reference

OSHA regulations set the legal floor. Consensus standards fill in the technical detail, and they carry weight in three ways: OSHA incorporates some by reference, cites others under the General Duty Clause, and owners write them directly into contract specifications.

Quality and Environmental Items in Contract Flow-Downs

A construction compliance checklist that only covers safety can miss important quality and environmental requirements in an owner’s audit. Public works and industrial owners flow quality and environmental requirements down through the contract, and a missing document there is a nonconformance just as surely as a missing guardrail.

Quality

  • ✅ Coating inspection records: surface profile, ambient conditions, DFT readings, holiday testing where specified
  • ✅ Material certifications and traceability from mill cert to installed location
  • ✅ Approved submittals on site and matched to installed materials
  • ✅ Inspection and test plan executed and signed, with hold points respected
  • ✅ Nonconformance reports opened, dispositioned, and closed with objective evidence

Environmental

  • ✅ Stormwater permit coverage (Notice of Intent filed) and SWPPP on site where required — generally sites disturbing one or more acres, including qualifying smaller sites that are part of a common plan of development or sale — requirements are set by the EPA Construction General Permit or the equivalent state program
  • ✅ Erosion and sediment controls installed and inspected on the permit schedule
  • ✅ Spill kits staged, spill response plan posted, fuel storage with secondary containment
  • ✅ Waste streams identified and disposal documented — including blast media and coating waste

If you are already ISO 9001 certified in the shop → your field quality items are extensions of processes you already control. The gap is usually that field records live in a truck, not in the QMS. Bring them under document control and the owner audit becomes a formality.


Inspection Frequency: What Gets Checked When

Inspection cadence is where checklists go to die. A single monster form nobody completes is worse than four short ones that get done. Split it by frequency.

Subcontractors and the Multi-Employer Exposure

Under OSHA’s multi-employer citation policy, more than one employer may be citable for a hazardous condition. A controlling employer — typically the general contractor — can carry responsibility when it has sufficient authority to prevent or correct the violation. Your checklist needs a subcontractor section:

  • ✅ Prequalification on file: EMR, OSHA logs, written program, training matrix
  • ✅ Site-specific orientation completed and signed before the first shift
  • ✅ Daily sign-in reconciled against training records
  • ✅ Subcontractor JHAs reviewed and accepted before work begins
  • ✅ Weekly compliance observation documented, with corrective actions tracked to closure

Objection worth addressing directly: “We are already OSHA compliant — another checklist is just more paperwork.” OSHA compliance is a legal floor, not a management system. It does not tell you whether the program is running consistently across five sites and three subcontractors, and it does not produce the evidence an owner’s prequalification or an ISO 45001 registrar will ask for.
